>Galbraith is due to undergo a medical in the Potteries on Tuesday after Swansea agreed to let the 25-year-old leave after only a year at the club.
>Galbraith, who can also play at right-back, was a standout performer for Swansea having joined from Leyton Orient for £1.5m.
>The Swans had been keen to hand Galbraith a new long-term deal this summer, but the former Manchester United youngster is instead set to move on.
>Swansea sources say it had been made clear that Galbraith was not prepared to sign a new contract.
>Galbraith's current Swansea deal runs until 2028, with the club having the option to extend it by a further 12 months.
>But Swansea have opted to sanction Galbraith's exit rather than forcing him to stay.
>The news comes as a hefty blow to head coach Vitor Matos given Galbraith's contribution last season.
